CurveBeam Standard Warranty Terms

CurveBeam, LLC warrants the purchaser that this hardware system will be free from defects for a period of one (1) year from the Standard Warranty Activation Date (typically the date of installation). During the standard warranty period, CurveBeam, LLC will correct any defects in material or workmanship, at no charge for material, labor, or travel. Any replacement parts shall be new or serviceable used parts and are warranted for the remainder of the original standard warranty or thirty (30) days, whichever is longer.

This standard warranty includes routine software maintenance upgrades, but excludes optional application modules.

Also included in the standard warranty is unlimited access to CurveBeam’s customer support team, (267-483-8081 Option 1) for general questions, accreditation, billing support, technical phone support, remote diagnostics, IT support, on-site service, repairs, replacement of defective parts (including full CT Tube and Flat Panel Detector coverage), and labor, which will be furnished without charge by CurveBeam, LLC.  Data back-up is not an included service and is the customer’s responsibility. Data recovery and external data transfer that requires customized scripts is not covered but assistance is available upon request (refer to F72-2.2 Price Sheet for pricing).

Service and maintenance will be provided during the hours of 8:00AM-8:00PM (EST/UTC) Monday through Friday, excluding US & UK country-specific bank holidays.

Warranty, service, and maintenance requested outside of service hours will be charged at the rates specified in F72-2.2 Price Sheet.

CurveBeam may require remote access for troubleshooting during the warranty period. If remote access is needed, and can not be provided, the travel and related expenses to have a CurveBeam tech (or designee) onsite are not covered by any warranty and will be the responsibility of the customer.

An annual preventive maintenance visit is recommended.  The included one-year standard warranty does not include a preventive maintenance at the end of the term.

Typical Service Response Time: One hour by telephone and next day for on-site support.

Warranty Expiration

Should you elect not to extend your warranty period, you may contract with CurveBeam, LLC on an as-needed basis (refer to F72-2.2 Price Sheet for pricing).

If a warranty contract lapses for more than thirty (30) days, the purchaser has the option to renew at a price 10% over the standard price.  A preventive maintenance visit in which the system will be assessed for damages is required prior to renewal.  A renewed lapsed contract will activate sixty (60) days after full payment is received.  Any technical support, labor, or replacement parts provided prior to activation of renewal will be billed at standard rates on an as-needed basis. If a warranty contract lapses for more than (120) days, the purchaser will forfeit their option to renew.

CurveBeam Extended Warranty Option

CurveBeam, LLC will extend the terms of the standard device 1-year warranty, which begins on date of installation, for up to an additional 7 years, as specified in the original purchase agreement, or subsequent warranty-specific purchase agreements (refer to F72-2.2 Price Sheet for pricing).

Extended warranty may be purchased at a silver, gold, or platinum level.  All levels include unlimited remote support.  All prices listed below are subject to change.  Please refer to F72-2.2 Price Sheet for current pricing.

Silver Warranty Option

All on-site service, maintenance, repairs, replacement of defective parts (including full CT Tube and Flat Panel Detector coverage), and labor is billed at $300/hour; and will be billed at a three-hour minimum. Technician travel and expenses are included. Response time is guaranteed within 72 hours, excluding weekends and holidays.

CurveBeam may require remote access for troubleshooting during the warranty period. If remote access is needed, and can not be provided, the travel and related expenses to have a CurveBeam tech (or designee) onsite are not covered by any warranty and will be the responsibility of the customer.

Replacement parts may be purchased at list price. See F72-2.55 Spare Parts List for pricing.

A preventive maintenance (plus additional parts & time as needed) visit may be scheduled for: PedCAT: $2,000, InReach: $2,000, LineUP: $2,500, HiRise: $3,500.

An additional 4-hour on-site training session, in addition to the installation training, for operators and/or physicians may be scheduled at a cost of $1,300.00.

Gold Warranty Option

All on-site service, maintenance, repairs, replacement of defective parts (including full CT Tube and Flat Panel Detector coverage), and labor is billed at $150/hour; and will be billed at a three-hour minimum. Technician travel and expenses are included. Response time is guaranteed within 48 hours, excluding weekends and holidays.

CurveBeam may require remote access for troubleshooting during the warranty period. If remote access is needed, and can not be provided, the travel and related expenses to have a CurveBeam tech (or designee) onsite are not covered by any warranty and will be the responsibility of the customer.

Replacement parts may be purchased at 50% of list price. See F72-2.55 Spare Parts List for pricing.

A preventive maintenance (plus additional parts & time as needed) visit may be scheduled for: PedCAT: $1,000, InReach: $1,000, LineUP: $1,500, HiRise: $2,000.

An additional 4-hour on-site training session, in addition to the installation training, for operators and/or physicians may be scheduled at a cost of $1,000.00.

Platinum Warranty Option

All on-site service, maintenance, repairs, replacement of defective parts (including full CT Tube and Flat Panel Detector coverage) and labor, is provided without charge by CurveBeam, LLC.

CurveBeam may require remote access for troubleshooting during the warranty period. If remote access is needed, and can not be provided, the travel and related expenses to have a CurveBeam tech (or designee) onsite are not covered by any warranty and will be the responsibility of the customer.

All replacement parts, including CT Tube and Flat Panel Detector, are provided free of charge. Shipping costs are not included for international customers.

One Preventive Maintenance visit, per year of warranty purchased, is included.

An additional 4-hour on-site training session, in addition to the installation training, for operators and/or physicians may be scheduled at a cost of $750.00.

All routine software updates are provided free of charge.

New protocols, features, and major updates are included or discounted. Information will be provided per release.

CurveBeam Warranty Exclusions

CurveBeam does not warrant or cover for faults arising from, but not necessarily limited to, the following:

  • Damage caused by impact with other objects, dropping, falls, spilled liquids or immersion in liquids
  • Damage caused by a disaster such as fire, flood, wind, earthquake, or lightning
  • Damage caused by unauthorized attachments, alterations, modifications or foreign objects
  • Damage caused by failure to provide a suitable operating environment
  • Damage caused by the use of the hardware system for purposes other than those for which it was designed
  • Damage from improper maintenance performed by other than OEM trained personnel
  • Damage from improper electrical connection or supply
  • Damage caused by any other abuse, misuse, mishandling, or misapplication
  • Damage to internal or external computer, software, or operating system caused by:
    • Applications other than its intended use.
    • Third party software or damage caused by unauthorized changes to the system software
    • Unauthorized upgrades, additions, deletions or unnecessary internet use, or any other unauthorized application.
  • Under no circumstances shall CurveBeam, LLC be liable for any special, incidental, or consequential damages based upon breach of warranty, breach of contract, negligence, strict liability or any other legal theory. Such damages include, but are not limited to, loss of data, loss of profits, loss of revenue, loss of use of hardware system or any associated equipment, cost of capital, cost of substitute or replacement equipment, facilities or services, down time, purchaser’s time, the claims of third party, including customers, and injury to property.

If the device is moved or serviced by someone other than CurveBeam, LLC, or designee, all warranty will become null and void.
